Equanimity Wellness, dba Albuquerque Taekwon-Do

Assumption of Risk, Release of Liability, and Medical Authorization

I, the undersigned student/parent or legal guardian, understand and agree to the following:

1. Assumption of Risk

   I understand that participation in Taekwon-Do and related martial arts activities involves physical contact and inherent risks, including but not limited to falls, sprains, strains, bruises, broken bones, and, in rare cases, serious injury. I voluntarily choose to participate / allow my child to participate and assume all risks associated with this training.

2. Release of Liability

   In consideration for being allowed to train at Equanimity Group, LLC, dba Albuquerque Taekwon-Do, I, for myself and/or my child, and for our heirs and legal representatives, release and hold harmless Equanimity Wellness, LLC, dba Albuquerque Taekwon-Do, its owners, instructors, staff, and agents from any and all claims or causes of action for injury, damage, or loss arising out of participation in classes, events, or use of the facility, except in cases of gross negligence or intentional misconduct.

3. Health and Fitness to Participate

   I certify that I / my child am in good physical condition and have no known medical conditions that would prevent safe participation in martial arts training. I agree to inform Equanimity Wellness, LLC, dba Albuquerque Taekwon-Do in writing of any changes to my / my child’s health or medical condition.

4. Medical Authorization

   In the event of an injury or medical emergency, I authorize Equanimity Wellness, LLC, dba Albuquerque Taekwon-Do staff to provide or obtain basic first aid and to contact emergency medical services if needed. I understand that I am responsible for all costs related to such medical care.

5. Acknowledgment and Agreement

   I have read and understood this waiver and release. I understand that by checking the agreement box on the registration form, I am giving up certain legal rights. I agree to abide by all school rules, instructions, and safety guidelines provided by Albuquerque Taekwon-Do instructors and staff.

Photos and Video

I understand that photos or video may be taken during classes or events. I grant Equanimity Wellness, LLC, dba Albuquerque Taekwon-Do permission to use images of me / my child for school-related promotional materials (print, website, and social media), without compensation.
